Coordinated servicesthe coordinated services department served 449 unduplicated client-partners identified as being in crisis and needing case management this fy. On 173 occasions, financial assistance was provided to client-partners and their families for a total of $30,403. 88. Coordinated services served 206 client-partners within case management and 47 client-partners received employment services within workforce development. Loaves & fishes: this program provides a 7-day supply of food to families via referrals by social workers, schools, clergy, medical staff, etc. The pantry provided food to 3,019 people this year (duplicated). Client-partners are eligible to receive food 8 times a year, with some exceptions. The pantry provided over 60,380 pounds of food, valued at over $100,230. 80. medical clinic: this clinic provided 1,756 medical office visits and nurse consultations in fy17-18, serving 388 individual (unduplicated) patients. Patients benefitted from primary and specialty medical care, mental health consultation, prescription assistance through nc medassist, donated medical supplies, free lab tests, diagnostic and specialty referrals, care coordination, etc. The value of medical office visits, based on 2017 usual and customary rate values provided by blue cross blue shield of nc, totaled $202,286 value prescription medications provided to our patients at no cost through nc medassist totaled $220,542. Dental clinic: the ajc dental clinic provided dental services to local patients 785 times and to regional patients 507 times in fy17/18. An additional 211 patients were screened via the dental clinic, and 71 received referrals and assistance with specialty dental care. The value of dental services provided to the dental clinic patients, based on 2017 usual and customary rate values provided by blue cross blue shield of nc, totaled $225,054. education services: the purpose of the youth education-learn works after school program is to foster academic development and advance family engagement for enrolled students and families. We served 54 students in 1st to 8th grade from 3 area schools. This summer we also piloted our mobile technology library and summer skill building program through our youth education-learn works program to maintain students academic progress. 19 students and their families benefited from the opportunity of having the center provide transportation home each day or otherwise would not have been able to participate in our programs. We also continued our partnership with the ymca and sunrise esl for morning adult esl classes to help adults improve in the english language. We served 26 adults between the 2 classes.
