Service/supports coordination is a service that provides the consumer/participant with a service/supports coordinator, who will assist the consumer/participant, with obtaining and maintaining services that are available in the home and community setting. These service allow the consumer/participant to live independently in their own home and to avoid potential placement in a nursing facility. A service/supports coordinator meets with consumers in their own home and provides advocacy and support. as an ohcds a bridge to independence coordinated non-medical tansportation and the purchase, installation, and payment of durable medical equipment and home modifications.
