Passport services: passport is designed to keep older adults at home or to help them return home after a nursing facility placement. Services provided on a non-discriminatory basis include: personal care, homemaker, home-delivered meals, medical emergency response system, medical transportation, and adult day care. Approximately 2,504 clients received passport services in 2014. assisted living: ohio's assisted living waiver program pays the costs of care in an assisted living facility for certain people with medicaid, allowing the consumer to use his or her resources to cover "room and board" expenses. The program is open to individuals currently residing in a nursing facility or enrolled in another home and community based waiver program, such as passport, choices or home care. It is also available to current residents of residential care facilities who have paid privately for at least six months. Participants must be financially eligible for medicaid and determined by the agency to need a "nursing facility level of care," the same requirements as the passport program. 196 clients received assisted living services in 2014. other program services: the other program services that are provided to older adults through area agency on aging, region 9's contracted service providers include: daily meal programs, day care, home making/personal care, health screening, transportation, escort, chore, home repair, case management, and care coordination program. The care coordination program is designed to link home care services to those vulnerable older adults who are at risk of losing their independence due to having multiple problems with daily living tasks and skills. 13,733 clients received these other program services in 2014.
