Services for independent living: provided more that 3711 hours of friendly visits to homebound seniors provided more than 2010 rides to 75 homebound seniors provided chore services to 25 families (257 hours) facilitate 5 rsvp bone builders classes attended by over 100 active seniors facilitate matter of balance (2) training workshops for at-risk seniors provided newspaper on tape provided individuals with counseling services (68 individuals, 305 hours) provided 72 individuals with homemaking services (1777 hours) facilitate tai j quan for better balance (fall prevention): 24 sessions caregiver support and care consulting: provided 3365 hours of respite services - relief for the caregiver provided caregivers with care consulting services (503 units) provided powerful tools for caregivers workshops for caregivers facilitated monthly memory loss support group for caregivers provided medicare counseling provided information, referrals and coordinated area resources effectively to enable seniors to remain in their homes independently nutitional and food support programs: provided food commodity assistance to 156 at-risk seniors in the city of aitkin (1588 boxes naps) delivery of more than 5391 hot meals for 55 seniors in the city of aitkin delivery of 3579 frozen meals for 31 seniors in rural aitkin county additional community collaborations
