Roadmap to health: care coordination for adult population of stark county. Referrals comes mainly from aultman er, community centers, churches and other nonprofit organizations. 406 individuals were served during 2016. thrive: meant to address health disparities in infant mortality in stark county, access health employs (through a grant from thrive) two community health workers (chws) and a chw program manager. We have oversight of the thrive community health workers with training, practicums, coaching/mentoring and a peer support learning community. The grant period is from october 1, 2016 - september 30, 2018.
