Ardent solutions, inc. Is a vibrant and passionate organization whose mission is to create synergy within and between systems, organizations, families and individuals that result in a strong culture of health and quality services for our communities. Founded in 1994, and incorporated in 2005, ardent solutions strives to be recognized as an industry leader harnessing all community assets to collectively impact healthcare priorities across the region. Ardent solutions proudly serves older adults, families and individuals living in poverty and those living with chronic health conditions, while supporting healthcare professionals and institutions by: strengthening accountability, empowering community health, increasing access to services, supporting the healthcare industry infrastructure, and enhancing community health planning. Ardent solutions works with partners to articulate clear goals, identify quantitative measures, select changes or interventions to help meet the goals, test the changes, study their effectiveness, and implement or modify the change activities. Through this process, ardent solutions leads health planning strategies, facilitates collaborative workgroups, implements evidence-based community interventions, and monitors and evaluates program effectiveness. Ardent solutions continues to develop innovative strategies to address our populations healthcare needs. Our projects focus on the following healthcare system priorities:strengthen the accountability of local providers for improvements in chronic disease management and injury prevention services;strengthen community health education and self-management skills to reduce costs of non-emergent ed visits, in-patient hospitalizations and improve the overall patient healthcare experience; increase access to quality mental health care through the development of a suicide safety-net integrating behavioral health and primary care systems;increase access to health care services through improved mobility options,increase the number of health care personnel trained and practicing in rural settings through provider continuing medical education programming and medical student housing; andenhance community health planning on a local and regional level. Activities align with the goals and objectives of the new york state department of health rural health network development program specifications including supporting our rural providers to improve access to care, enhance coordination of services, increase efficiency of service delivery, and coordinate resources to decrease duplication and achieve cost and operational savings. Some examples of activities within these strategies include, but are not limited to: a. As an attested partner in the regional dsrip and a member of wny phip, ardent solutions provides a rural perspective on key health care issues and opportunities for system improvement within the nys medicaid redesign program and nys prevention agenda. We work diligently to engage medicaid recipients and/or those who are un-insured to become knowledgeable about their health conditions, provide training on self- management techniques and strategies to stay healthier when living with a chronic health condition, facilitate one-on-one health coaching for personalized interventions and education, and conduct health activation assessments to learn better ways to engage patients. B. Leading efforts to improve the environmental quality and the physical environment of older adults wishing to remain independent in their home, resulting in the decrease of unintentional injuries leading to emergency department visits, hospitalizations and out-of-home placement. Ardent solutions has developed comprehensive fall prevention initiatives encompassing evidence-based education programming, evidence-based strength training programs, and medication management services.
