Aids community resources, inc. D/b/a acr health, is a community-based organization providing prevention, education, and support services to people with chronic illnesses, including hiv/aids, and those at risk for contracting hiv, stds, and hepatitis infections. The agency helps create safe spaces for people by offering tailored education and support systems for marginalized populations, such lgbtq individuals and people who use drugs, in order to help individuals gain access to and fully utilize/navigate healthcare systems. Direct services include the prevention, youth, care management, insurance and support service departments.
Education
Support services:to provide direct support services to individuals living with chronic illness, including hiv/aids, substance use disorders and/or mental health issues, through care management, insurance enrollment, transportation, supportive housing, nutrition health education, employment assistance, emergency assistance, information, referrals and advocacy. prevention:to prevent hiv/std/hcv infection and increase community awareness through education, outreach and engagement services. This includes: syringe exchange programs (fixed and mobile), opioid overdose prevention activities, linkage and navigation to treatment and care, on-site acute medical care, hiv/std/hcv testing, prison based education and transitional planning, street outreach, lgbtq youth services and referrals.
